1. Schedule yourself for a chart update by typing this link into your browser: https://app.elationemr.com/book/chartupdate Link is available all hours of the day/night. You can schedule and complete now OR schedule ahead with reminders to complete. The link is smartphone compatible but is viewed better on the computer. 
2. Fill in demographics with physical address and PO BOX (if you have one) and update other info (insurance if applicable etc)
3. Enter family medical history, personal medical history, medications and health goals for the year. Please include all medical information. No need for specifics (that’s what visits are for). But be brief and include year-of medical issues when possible.
4. Submit and sign up for your echart patient passport for easy communication and access to records.
