Start with an intake and clearance consult for $150 then choose your Provider assisted program....
-These plans do not apply to Medicare B original patients as your appts will be billed to Medicare.
-Medicare patients will pay for the medication only (not covered by Medicare B -- monthly $ via insurance found to be $600/mo).
-These plans are in addition to DPC Primary Care membership plans. The initial consult is included at no additional cost for current Thrive Direct members all member types.
-Medication choices are in addition to the program fees unless covered by insurance.
-Extension/weaning program available after the 6 month program completion
-Small Group Nutrition Sessions ... may be coming back soon ??
