OPEN for House Calls

​I am currently only doing House Calls ONLY until a clinic location is up and running. Please email or call me for availability. FEE $60 unless otherwise posted. Insurance is not accepted. 

House call at this time is available to all patients due to no physical clinic location. Visit types will be limited to Pain Management, Complex Care, Acute Care/Urgent Care, Sports Physicals ($40), DOT/CDL Physicals ($125) and minor office procedures (a la carte-ask for pricing).
