Please print and fill out/sign the below forms. If the student fills out the form (it is actually a good skill to teach)- parents are to be sure to review for accuracy. 

If the student is a minor parents are required to sign. 

Parent does not need to be present and must sign the additional consent to not be present. Clothes stay on the entire time and there are NO hernia checks.
